At the time of your appointment, you will be asked to complete a Patient Registration Form and a Health History Form. These can be completed or obtained online. If we mailed them to you, please complete them in full and bring them with you to your appointment. In addition to your health history form, please review your pre-surgery packet. Be sure to follow the guidelines outlined for surgery preparation.
Please make sure that you bring the most current insurance cards with you to your appointment as insurance companies frequently modify benefits. If your insurance information, address or telephone number change at any time throughout the course of your care with us, please notify us promptly.
