Online Appointment Request
We understand that life is chaotic and sometimes fitting a doctor appointment into your already packed schedule is difficult. As a convenience, we’re glad to offer this online appointment request form. Please fill out your contact information and available appointment times and an Omni representative will contact you within 24 hours.

Phone Appointment Request
If you prefer to phone in an appointment request:

  • Call us at 303-377-2020.
  • When one of our representatives answers the phone, please let them know that you would like to speak to an appointment coordinator to schedule your appointment or surgery.
  • When you speak with an appointment coordinator, please have your insurance card out and handy to reference.
  • If you are new to our clinic, we will send you a packet of information. In the packet are forms for you to fill out. They include a patient registration form and patient history form.
  • We ask that you bring these forms to your appointment and to arrive 15 minutes before your scheduled time 

If your primary eye care doctor or primary care physician has referred you to us, we will communicate with them to ensure you receive  continuous quality care.
